1. First and last name of the new user (e.g. John Smith)
2. Preferred username (e.g. john.smith)
3. Preferred new email address (e.g.  john@mydomain.comThis e-mail address is being protected from spambots. You need JavaScript enabled to view it )

For changes or removing an address, please provide the username and the details of the request.
